ATLS 8th Edition - updates / suggestions

Just a reminder that suggestions / additions / modifications for the 8th
edition of the ATLS Provider course manual are due by the 15th November.

We've added a Wiki page where you can review/comment on suggestions so far
and, of course, add your own.
