well, while it is a side effect of the prohibition of physician 'collusion  
and price fixing' (under both civil and criminal statues), it is the bare  
truth.
 
The way things have worked out in my professional life time has been that  
the excesses of the freewheeling 1970s lead to more and more power being  
consolidated in the hands of the petty beaurocrats of Medicare, Medicaid and the  
various giant health insurance consortiums starting in the late 1980s with the  
rise of the HMOs.
