Octreotide's primary use seems to be to decrease volume in high output
fistulae,(which can be helpful with regard to wound care as well as
fluid and electrolyte issues) but it does not seem to decrease the rate
of spontaneous closure or the ultimate duration of the fistula in most
literature reports....
We preach to the residents that in the majority of situations, the
cornerstone of management should be adequate drainage, infection
control, evaluation of obstruction or stricture, and nutrition rather
than the octreotide..hence, we rarely use it.
