The obsolete Trapdoor

Hi,
   
  we discussed this again and thought that the best incision in terms of access and morbidity would be a high anterolateral thoracotomy (would give access to proximal SC vessels and probably to L chest as well). I agree that trapdoor is no obsolete, but is very morbid. Is there any standardized approach at all to such injuries?
