3T because the GCS doesn't go lower than the numeral 3. Just like you can't have someone with a GCS of 16.
RE: your debridement question: in my practice (extensively wounds) if it doesn't jump (and there are no chemical paralytics on board) it comes out. Dead muscle is a recipe for disaster.
