Fluid overload and heart failure are obvious causes. Was he  
digitalized? Arrythmias and atrial fib are common post pneumonectomy.
I had a gun shot victim recently who developed aspirgillosis empyema  
on the right pneumonectomy side and a broncho-pleural fistula. Two  
attempts at bioglue were unsuccessful and it was reclosed with a  
muscle flap. Complications and mortality are high in traumatic  
pneumonectomy patients.
